REQUIREMENTS

  • Completely filled up Application Form.
  • Original Passport (Should be valid for more than 6 months).
  • 2 pieces of 2×2 picture.
  • Bank Certificate (Must provide proof that you have enough money for your visit to US).
  • Income tax return (ITR).
  • Land Titles, Car Registration, Stocks etc.
  • Employment Certificate and Application for Leave of Absence / Business Registration.
  • Photocopy of Credit Cards / Statement of Account.
  • Proposed itinerary.
  • Invitation letter (From your Sponsor / Relative / Friend. If applicable).
  • Marriage Certificate / Birth Certificate (NSO Authenticated).

Processing Fee

  • PHP 15,000.00

  • Approximately 15 working days from the time application is received by the embassy.

 

Visa Fee 

  • USD 160 (Subject to change)

  • Please do not leave any blank in the application form.
  • Kindly put “N.A.” if it’s not applicable.
  • Make sure that the back page of the passport is completely filled-up (Address, Tel Numbers and Name of Person to be contacted in case of emergency.
